Ask a new Star Rail player which slot they can safely cheap out on, and most will point at the sustain. It is the wrong answer, and it is the most common reason accounts stall on the last floor of an endgame mode.
Here is the thing the tier lists that only rank carries never tell you: in modern Honkai: Star Rail, a sustain is not a healer with a heal button. The best ones advance turns, generate energy, cleanse debuffs, break enemy toughness, shred defense, or deal real damage while they keep you alive. A great sustain buys your carry more actions, feeds Break, or covers the one mechanic a floor is built around. A bad one just barely keeps the party breathing while contributing nothing else, and that gap is worth a full clear.
So this ranks HSR sustains by total value, not raw healing numbers, for the Version 4.5 meta. Below each tier is the kind of team it belongs in, because the right sustain is the one that fits your carry, not the one with the biggest heal. Pair this with the full HSR tier list for where each unit sits overall, and the endgame mode guide for what each floor actually tests.
Why "just keep me alive" is the wrong bar
Every fielded unit in Star Rail occupies one of four slots. A sustain that only heals is spending a quarter of your team on survival and nothing else. The units at the top of this list refuse to do only one job:
- They pay you back in tempo. Turn advancement, speed, and energy from a sustain turn into extra carry actions, which is damage.
- They cover a mechanic. Cleanse for debuff floors, toughness for Break teams, defense shred for a carry that wants it. The sustain is often where a team's answer to a gimmick lives.
- They ask for little in return. The best sustains do not fight your carry for skill points or energy, so they slot cleanly instead of warping the rotation.
Rank a sustain on that, not on the heal bar, and the order below falls out naturally.
S-tier: sustains that carry their weight twice
These do their sustain job and a second one, and they fit almost any team you build around them.
- Aventurine (Preservation). The benchmark shielder. He shields the whole team off his own defense, revives on a threshold, feeds follow-up damage, and brings crit value to teams that stack it. On any FUA or crit-leaning comp he is not just the tank, he is a damage contributor. If you own one premium sustain, this is usually it.
- Huohuo (Abundance). The premium healer standard: strong healing, debuff cleanse, and a large ATK buff plus energy regeneration on her Ultimate. That energy is the quiet part that matters, because feeding your carry's Ultimate faster is damage. She lifts almost every DPS in the game.
- Hyacine (Abundance). The Version 4.5 modern healer, built around keeping speed and energy high while she sustains. A healer who does not slow the team down and keeps the Ultimate economy flowing is exactly what a fast Break or Super Break team wants in the fourth seat. She arrived in 4.5 and immediately became a top-tier pick.
- Lingsha (Abundance). Healing plus a follow-up damage engine plus toughness reduction. On a Break team she is close to mandatory, because she sustains and feeds the exact damage type the team runs on. The best-in-slot Break healer.
- Fu Xuan (Preservation). Team-wide damage reduction, a crit rate boost, and a revive tied to her HP pool. She is the survivability answer for greedy hypercarry teams that want to face-tank a boss and keep hitting. Where Aventurine leans into follow-up, Fu Xuan leans into raw endurance.
A-tier: excellent, slightly more conditional
Still first-rate; each is a touch more specialized or asks for the right team.
- Luocha (Abundance). Nearly automatic healing and cleanse from a field that triggers on its own, which frees your attention and skill points. Older, but still one of the most low-maintenance sustains in the game. Perfect when you want to forget the sustain slot exists and just play the carry.
- Gallagher (Abundance). The four-star that punches at five-star weight on Break teams. Cheap to build, feeds Break, heals plenty, and applies a debuff that boosts the damage enemies take. If you do not own Lingsha, Gallagher runs the same Break teams at a fraction of the investment. The single best F2P-friendly sustain in the game.
- Gepard (Preservation). A big, reliable team shield plus Freeze control. He does less than Aventurine or Fu Xuan on the offense side, but on a fragile team that just needs a wall, he is rock solid and asks nothing complicated of you.
B-tier: gets the job done, especially early
- Bailu (Abundance). Strong healing with a built-in revive, though no cleanse and a slightly dated kit. Perfectly clear-capable, just outclassed on utility by the S-tier healers.
- Lynx (Abundance). A four-star healer with a taunt and HP scaling that shines on HP-based carries and Preservation-adjacent teams. Niche but genuinely good in her lane.
- March 7th (Preservation). The free early-account shielder with a Freeze counter. She will carry you through the early equilibrium levels and holds up longer than people expect on a budget team.
- Natasha (Abundance). The starter healer. Free, reliable, and completely fine until you pull or build anything above her. No shame in running her while you find a better option.
C-tier: bench unless the account demands it
- Preservation Trailblazer and the older free sustains. Serviceable in a pinch and occasionally clever on a specific gimmick floor, but you will replace them the moment a real sustain unlocks. Keep one leveled for the rare floor that punishes their absence, and otherwise leave them benched.
Match the sustain to the team, not the tier
The number-one placement on a tier list is not automatically the right pick for your account. Use this instead:
| Your team | Best-fit sustain | Why |
|---|---|---|
| Break / Super Break | Lingsha, Gallagher, or Hyacine | They feed Break or preserve speed and energy while healing |
| Follow-up / crit hypercarry | Aventurine | Shields, crit value, and follow-up damage in one slot |
| Greedy single-carry (Acheron, boss duels) | Fu Xuan | Team-wide damage reduction and a revive keep the carry hitting |
| Debuff-heavy floor | Huohuo or Luocha | Cleanse is the mechanic answer, and both heal through it |
| Low-attention / auto play | Luocha | Field-based healing needs almost no management |
| Early or F2P account | Gallagher, March 7th, Natasha | Cheap, effective, and clear-capable right now |
A sustain that fits your carry and covers the floor's mechanic beats a "higher-ranked" one that does neither. That is the entire art of the fourth slot.
The takeaway
Sustain is not the slot to save money on. It is the slot that decides whether your expensive carry gets to keep hitting. The S-tier picks all share the same trait: they keep you alive and do a second job, whether that is tempo, Break, crit, or damage reduction. Build one great sustain per archetype you run, match it to the fight, and the last floor that kept timing you out starts falling.
