Sanity (SP) is a per-Sinner stat ranging roughly from -45 to +45. It directly affects the coin heads-rate: the higher a Sinner's Sanity, the more often their coins flip heads, which makes them win Clashes and deal more damage.

Winning clashes and good encounter outcomes raise Sanity; losing clashes and allies dying lower it. At very low Sanity a Sinner risks entering a Panic state, losing control for a turn. Managing Sanity, especially before firing E.G.O, is a key skill in harder content.
